Insurance & Payment Policy

We accept most insurance plans. We will assist in filing your insurance; however, your insurance is a contract between you and the insurance company.   We encourage you to check your insurance benefits prior to your visit with us.

We ask that patients obtain any required referrals from their primary care physician prior to their scheduled appointment and bring a copy to their appointment.

Insurance co-payments are required at the time services are rendered.

Should you require surgery, our staff will make every effort to see that your insurance company has been notified and that all precertification requirements have been met. We encourage you to communicate with your scheduler to insure this has been handled, if applicable.

Some surgical procedures may not be covered by your insurance company in which case you would be responsible for payment in full prior to surgery.

Should you not have insurance, a deposit for your office visit is required.

Visa, Mastercard, American Express, Cash, and Personal Checks are acceptable forms of payment. There is a $25 fee for checks returned due to insufficient funds. Please note that when you have surgery, separate bills will be sent by the Operating Room facility, Pathology, Radiology, and Anesthesiology Services. Questions regarding their fees should be addressed to the provider of each of those services.
